Section 1: Forms that Must be Returned

These are the forms that your recruiter MUST have back before you begin your assignment:

1. Volunteer Travel Information Form

2. Volunteer Release – US citizens

3. Addendum to the Volunteer Release

4. Medical Clearance Form

5.  Banking Information Form

6. Supplier form

7. Scanned copy of your passport and COVID-19 vaccination card: Email to Emily Keast

8. Benin References Form – please send this form to 3 of your chosen references

Section 2: Essential Documents

• Benin Visa Guidance 
  • Email Emily Keast that you have your eVisa and have printed it.
• US Volunteer F2F Expense & Reimbursement Form
  Note: Please read the above document in its entirety since it includes important information on what costs CRS covers directly, reimburses volunteers for, what are non-reimbursable expenses, and how to submit your reimbursements. CRS is not responsible for any volunteer costs incurred that are not in compliance with the information shared below.

Section 3: Insurance, Safety & Security

Please review the information in the links below once your assignment is confirmed and at least two weeks prior to your departure date. These important documents include information about the situation “on the ground” and your insurance processes so that you are best prepared for your assignment.

• CRS Learns Courses: There are two short courses that volunteers are required to take prior to departure. Please follow the steps below to enroll and complete the courses.
  1. 1. Fill out and submit this form: CRS Learns Self-Registration Link
     2. Inform your recruiter (Email to Emily Keast) once the registration is submitted – It will take 48 hours for you to gain access to the courses
     3. Complete the Introduction to Safeguarding and Safe & Sound Courses assigned to you
